Universal Studios Code of Conduct & Medical Consent Form

GRAD NIGHT 2017 will be a sensational evening filled with lots of fun, rides, food & entertainment. It will be a memorable celebration that the senior class of 2017 will not want to miss! There will be dining, dancing, rides, attractions & live entertainment. We want our kids to have a terrific evening, but we also want them to be safe. 47% of traffic fatalities of 15-20 year-olds are alcohol-related and graduation night is one of the most dangerous nights. By including bus transportation, we minimize the driving dangers of the evening.

Thursday, June 1, 2017 @ 8:30 pm, returning Friday, June 2 @ approx. 2:30 am.
Seniors will check in at LCHS at 8:30 pm. Chartered buses will depart at exactly 9:00 pm and will not be held for late arrivals. Buses will return to LCHS at approximately 2:30 am. Please arrange transportation home for your senior, as lack of sleep could make driving home unsafe.
Note: Since round-trip bus transportation is provided, self-transported students will NOT be admitted, for security reasons.

GRAD NIGHT 2017 will be held at Universal Studios and promises to be a blast! The venue will be closed to the general public – graduates of LCHS and other high schools will have exclusive access to the park. It has something for everyone. We will start the evening with an all-class private dinner @ Jurassic Park. There will be DJ’s & dancing, a behind-the-scenes studio tour, rides and attractions including Fast & Furious – Supercharged and The Wizarding World of Harry Potter experience. All food venues will be open for additional purchases throughout the evening.

What’s Included:
Round-trip bus transportation from LCHS, a private dinner buffet where we will raffle off amazing gifts and prizes, a California Neighbor Pass for full access to rides and attractions, DJ’s, dancing and entertainment for the whole night AND the pass is valid to revisit the park for 9 months from Grad Night event date (blackout dates apply).

Cost, Payment and Forms:
$150 per person. All seniors in the LCHS Class of 2017 are invited to reserve their spot for Grad Night 2017 now through May 15. Reservations received after May 15 are $175 per person and will be placed on a first come/first served waiting list and are not guaranteed. Reservation Form, Parent/Student Waiver, and Universal Studios code of conduct and medical consent must be signed and submitted with your payment by May 15, 2017.
Make checks payable to “LCHS PTSA Grad Night” and put the student name on the memo line.

If ordering online, please mail or drop off reservation & waiver forms as noted above.

Once purchased, tickets are non-refundable and non-transferable.

Security will be provided at check-in at LCHS, on the buses, and at the venue. We will also have La Canada parent chaperones on the buses and at the venue.

Note: Grad Night 2017 will be a safe, supervised, secure; drug and alcohol-free alternative to graduation parties and all students attending will be expected to adhere to a code of conduct.
